Dear Patients:
We are writing to inform you that Dr. Neavyn is leaving our practice. Her husband has accepted a position in Maine and they will be moving there this summer. We assure you, however, that Dr. Ling and Dr. Williams will be taking over her patients and will continue to provide excellent care. We also will be bringing on another optometrist and are hoping to hire another ophthalmologist in the near future as well.
We will be happy to continue your care here. All medical records and testing will be made available to your new provider within the practice. Of course, you may choose a different ophthalmologist from the community. If this is your choice, you can contact your insurance company to obtain a list of ophthalmologists in the area who accept your insurance and once you have established care, we can provide you with a medical record release form to forward your records.
We understand this news is coming at a very difficult time. We would like you to know that we are using this unprecedented time during the coronavirus pandemic to better our practice. We are looking at ways to improve efficiency as well as to update our practice. We greatly value our relationship with you, and hope that you will choose to continue receiving your care from us.

I am so sorry to have to leave at such a difficult time. It has been an honor to provide your care these last few years. This move is an unexpected one, but it is a wonderful opportunity for my husband and I know it is what is best for our family. I am going to miss every one of you and I am so glad that I have had the opportunity to know you. I am absolutely confident that you will be well taken care of by my colleagues, Dr. Ling and Dr. Williams. They are caring and thorough physicians who will make sure that your conditions are managed and monitored closely. I wish you all the best of luck. Please stay safe and well during this crazy time.
